Regarding Appointments It is the patient's responsibility as well as ours to arrange a time that is mutually convenient. We try hard to maintain our schedule so you will not have to wait when you arrive for your appointment. We will complete your treatment as soon as possible. We ask that you arrive on time. When unable to keep an appointment, 24 hours notice is required. Failing to notify this office 24 hours prior to missing your appointment will result in a charge for the missed appointment. New Patients For your convenience, you may download and print the form needed to register you as our patient. Please fill out this 2-page form completely and bring it with you for your first appointment.
